View Submitted Forms

  • In order to view submitted forms you must be logged into your Portal account
  • Click on your user name
  • This will open a menu choice
  • Click My Forms
  • Go to View submitted forms
  • All submitted forms will display in a table.  To view a submitted form click the form title to open.
