Gateway Programme Referral

Please be aware, this form is to make a referral to the Gateway programme, it is NOT a referral into Domestic Abuse Intervention & Prevention Services (DAIPS). Following your referral to the Gateway Programme being screened and accepted course facilitators will contact the client directly to discuss their individual needs and requirements before offering them a place. If you wish to discuss the referral with facilitators once it has been accepted, please call the duty line on 0300123 7047 and choose option 2. We will assume that following x3 failed attempts to contact the client that they no longer wish to attend the course and the referral will be closed. In the event of the referral being closed you will be notified via email. Please tick the box to confirm you have read this.

Please note the programme will be offered twice, and if not accepted then a new referral will be needed. If you are requesting 1:1 Gateway, then please be mindful sessions are very limited due to capacity and the timescales for the course will be longer.

The team can be contacted 9:00am to 5pm from Monday to Thursday and 9:00am - 4.30pm on a Friday. If you have an urgent concern outside these hours, or over a bank holiday, please call the Emergency Duty Team (out of hours) on 01244 977277.

If you or the individual you are completing this form on behalf of is at significant and/or immediate risk, please contact the Police via 999.
