It is the responsibility of all Foster Carers to report any specific incident which might occur in respect of every Child/ Young Person who is placed with them.
A report should also be made if the specific incident, concerning any Child/ Young Person who is accommodated by the Local Authority, comes to the attention of a Foster Carer.
Concerns which need to be reported include:
- any illness/ injury which requires medical treatment
- self harm
- substance and alcohol misuse
- missing from home
- theft
- criminal damage
- involvement in criminal activity
- bullying (bullying or being bullied)
- incidents of physical restraint
- any other significant issues
NB. Any serious incident should always be reported to the Child/ Young Person's Social Worker, and their Supervising Social Worker as soon as possible. Initially, this can be done by telephone, but should always be confirmed by completion of a Specific Incident Form.
